Details for medical professionals about how to refer a patient for hydrotherapy or physiotherapy.
If you would like to refer a patient for physiotherapy, it is helpful for us to receive as much information as possible prior to their assessment. This can be provided in a letter format or via our referral forms.
Once we have received the information, our reception team will contact the patient to book an appointment. Alternatively, the patient can contact us direct to arrange an appointment with a member of our therapy team. The patient should bring along any relevant information they may have.
When to refer
If you are a physiotherapist, chiropractor or osteopath, we do accept direct referrals into hydrotherapy if the patient meets the following criteria:
- the patient is currently having treatment with the referring health professional for the same problem to be treated in hydrotherapy
- the referring therapist has assessed that hydrotherapy is an appropriate treatment for the patient and has completed and signed the health screen form
- our referral form has been fully completed by the therapist, with a signature and contact details on the referral
Completed forms should be posted to:
The Physiotherapy Centre
Holy Cross Hospital
or you can fax to 01428 644007
The forms need to be sent to us prior to the patient's appointment. If you have any queries about our referral process or services, please contact us.
